2. How to Participate

Qualified participants who complete the survey are eligible to receive a $50 Amazon eGift Card after a brief one-minute phone verification. The eGift card will be emailed the same day the verification call is completed. Verification calls must be scheduled and completed within 14 days of survey submission to claim the incentive.3.
